Lot Division
| Falls and Frailty Services and Solutions This lot aims to provide a full solution for the provision of Falls and Frailty services including prevention, education, and training. Falls and frailty teams join up care by connecting hospital expertise with emergency services and use technology to reduce the risk of falls by remotely monitoring patients. The team typically attends to patients who have had a fall or are displaying related symptoms associated with frailty. They can assess, treat underlying illness, and minor wounds, assess mobility and function, and provide appropriate equipment to enable them to remain at home safely, with wrap around support provided where needed with the hospital at home. |
| Urgent Community Response Services This lot aims to provide a full solution for the provision of Urgent Community Response Services. 2-hour Urgent Community Response (UCR) is a team of health and social care professionals who help to maintain people at home - preventing avoidable hospital visits and stays - and assist people returning home from hospital who need extra support. They provide an urgent assessment within two-hours, and further multiagency support within 48 hours, for a short time. This is to help people recover quickly and keep their independence. |
| Adult and Peadiatric Acute Respiratory Infection (ARI) Hubs This lot aims to provide a full solution for the provision of Adult and Paediatric Acute Respiratory Infection (ARI) Hubs. ARI Hubs aim to support respiratory patients with urgent clinical needs by enhancing same day access to assessment and specialist advice as needed. This may include accessing secondary care respiratory 'hot clinics', access to point of care (POC) testing or other diagnostics may be helpful if available. Referral to Same Day Emergency Care (SDEC) services may also be utilised to support further diagnostics where required. This may be followed by advice, treatment, follow-up appointments or monitoring as required such as referral to an ARI virtual ward. |
| Reablement Services This lot aims to provide a full solution for the provision of Reablement Services. Reablement is a type of care that helps patients relearn how to do daily activities, like cooking meals and washing. Most people who receive this type of care do so for around 1 or 2 weeks, although patients can get free, short-term care for a maximum of 6 weeks. |
