Questionnaire to inform and assist the development of a future service delivery model for Ageing Well Southwark.
Total Quantity or Scope
Ageing Well Southwark is a preventative, community-based service for older people and carers in Southwark. Currently, Ageing Well Southwark delivers the following services: 1. An information and advice service for older people and carers. 2. A facilitation service, which is similar to social prescribing. The service connects older people and carers with their wider community and supports their independence and wellbeing via signposting, referrals, and/or one-to-one personalised support. 3. A range of groups, activities and services for older people and carers that could improve their health and wellbeing and/or prevent, reduce, or delay their care and support needs. Examples include befriending, exercise classes, creative sessions, IT support, peer support, and the ‘Happy Feet’ toenail cutting service. In addition to the above services, Southwark Council would like to explore commissioning an integrated ‘front door’ for the Adult Social Care Older People and Physical Disabilities service. The intention is to design a service that identifies where residents’ needs would be best met by the Ageing Well Hub, rather than statutory services, and supports these residents accordingly. Southwark Council is keen to engage with providers in the design of a future model for Ageing Well Southwark. We would like to hear from any organisations that have experience providing these services to further understand: • Interest in working in Southwark. • Views on service components, including potential challenges, issues and/or risks. • Preferred service and contracting models. Interested providers are requested to complete a questionnaire (appendix A) and to return it via the messaging service on the Proactis portal, not later than 5pm on Friday 12th January 2024.
