Description

The Specialist Floating Support service will: • Support adults with multiple disadvantages stemming from a combination of needs that would include mental ill-health, substance misuse, forensic background and behaviours of distress and who as a result find themselves at risk of becoming homeless or who are homeless. • Provide personalised and coordinated approach to the support of individuals. • Support individuals to achieve their outcomes and promote their independence. • Work with existing community groups part or outside the Open Mental Health Alliance and system partners such as Housing, Health, Adult Social Care and Probation services.

Interested organisations must be able to meet the following requirements: • Have a base in Somerset • Have an extensive experience in supporting people with multiple disadvantages including people with complex mental health needs, trauma, substance misuse and offending history. • Be skilled in supporting people who can be reluctant to engage with services • Support a trauma informed approach to practice. • Provide robust support to a staff team who will be working with people with complex and challenging situations and stories • Be willing to work in partnership with other community- based and statutory organisations to enhance the support offer to individuals • Be prepared to be part of developmental work and the need to adjust the model of support and align the service to the changing landscape of mental health and housing services?
