Description

NHS Shared Business Services Limited (NHS SBS) act in an Agency capacity for and on behalf of its customers (Approved Organisations) - existing and new. These comprise of NHS and Social Care organisations (whether acting individually, or on behalf of, or together as members of any consortia) along with any other public or private sector bodies which NHS SBS authorises to use the resulting Framework. NHS SBS intends to put in place a Framework Agreement for the provision of Admission Avoidance and related Goods and Services for Healthcare to be used by NHS SBS Approved Organisations. 6. II.2.4 Description of the procurement

This lot The framework agreement aims to provide a full solution for the provision of Falls and Frailty services including prevention, education, and training. Falls and frailty teams join up care by connecting hospital expertise with emergency services and use technology to reduce the risk of falls by remotely monitoring patients. The team typically attends to patients who have had a fall or are displaying related symptoms associated with frailty. Virtual Wards/Hospital at Home Support Services. They can assess, treat underlying illness, and minor wounds, assess mobility and function, and provide appropriate equipment Virtual wards/Hospital at Home Support Services allow patients to enable them receive the care they need at home, including in care homes, safely and conveniently rather than in hospital. This procurement is for a managed service for patients to remain receive care at home safely, with wrap around to help prevent unplanned admissions to hospital. The virtual wards service will be developed to support provided where needed with the hospital increasing Urgent and Emergency Care (UEC) operational pressures, reducing demand for acute bed capacity and improving patient flow. This can be a virtual ward for frailty, cardiology, respiratory, epicentre (emergency point of care testing and treatment without transfer to hospital), palliative, paediatric and any other virtual ward service that may be appropriate to provide care to patients at home.
