Description

NHS South West London CCG on behalf of all London CCGs is lead commissioner for a Dynamic Integrated (Urgent) Care Plan (UCP). The UCP is predominantly a software service that enables the sharing with urgent care services (111, London Ambulance Service, Emergency Departments in Acute Trusts) of treatment escalation plans, crisis care plans, end of life care plans, patient specific protocols and personalised care plans that have been co-developed and documented by different health and care disciplines. There were 50,588 Londoners, 0.5% of the London population, who had a co-created urgent care plan documented on the existing software service in December 2020. Of these, 4324 were viewed 9704 times by urgent care services in that month.

Total Quantity or Scope

On behalf of all London CCGs, the lead commissioner of the service, South West London (SWL) CCG, is seeking an innovative, collaborative development partner or supplier to build on the existing and emerging care planning and digital initiatives to transform the way care plans are captured and made available to patients and the health and social care system. We are seeking a partner that will develop enhanced access, interoperability and dynamic care planning functionality that enables continuous (urgent) care planning across multiple clinicians in multiple organisations. The solution should build on the existing capability and contribute to and interact with information available in the One London Health and Care Data Service (the London Care Record).
