Legal Justification
Children with specialist palliative care needs require an individual package of care in a planned, co-ordinated, timely and flexible manner as directed by need. The Health Boards require additional services to act as an adjunct to those which they deliver as part of core services in order to support planned and emergency respite and end of life care based with Ty Hafan, a specialist children’s Hospice provider. The service will support children, young people and their families and provide a palliative care experience within the community and home where necessary.
