Description
Derby City Council is inviting Applications to establish a Preferred Provider List (PPL) for Public Health Local Health Services. These statutory demand led Services are to be delivered within the City of Derby and are split into the following Services and Lots: GP Services • Lot 1 - NHS Health Check Programme (approx £180,000GBP per annum) • Lot 2 - Provision, fitting, monitoring, management, and removal of Subcutaneous Contraceptive Implants (approx £110,000GBP per annum) • Lot 3 - Provision, fitting, management, and removal of Intra-uterine Contraception Device Fittings (IUCD) and Intra-uterine System (IUS) (approx £85,000GBP per annum) • Lot 4 - Primary Care Child and Family Weight Management Extended Brief Intervention Service(approx £60,000GBP per annum) Service Providers/Clinicians delivering the Service must be a registered member of the UK/Irish General Medical Council (GMC) or Royal College of Nursing (RCN) and have a license to practise, if required by your regulator. Pharmacy Services • Lot 5 - Pharmacy Based Needle Exchange (approx £60,000GBP per annum) • Lot 6 - Community Oral Emergency Contraception Services (approx £9,500GBP per annum) - Service Provider must be Registered Health Professional in accordance with National PGD regulations • Lot 7 - Supervised Consumption (approx £355,000GBP per annum) Service Providers/Pharmacies delivering this Service must be registered with The General Pharmaceutical Council (GPhC). Applicants are must be able to meet the minimum requirements of the Specification(s). Organisations with multiple Service delivery sites must complete an application per site (individual Applications). This means if you are a Group applying on behalf of various sites, that each GP Practice or Pharmacy site will require its own unique Login and Email Address for East Mids Tenders to submit separate individual Application(s). Applications solely on behalf of a Group will not be accepted. The initial Contract period is five years (5) the contract is estimated to commence 01 April 2022 and expire 31 March 2027. There will be an option to extend the Contract for two (2) years in annual increments, subject to satisfactory performance and business needs. Maximum Contract period is seven (7) years. The Total Value of this Contract is £6,016,500 over the maximum seven (7) year period.

1 | NHS Health Check Programme
This service aims to improve the health outcomes and quality of life amongst Derby City residents by identifying individuals at an earlier stage of vascular change. It provides opportunities to empower them to substantially reduce their risk of cardiovascular morbidity or mortality. In turn this will lead to a reduction in the incidence of acute cardiovascular events in the Derby population. • To offer an NHS Health Check to 20% of the eligible population every year with an uptake level of 50-75% • To enable the early detection of hypertension • To enable the prevention and early detection of diabetes • To enable the early detection of chronic kidney disease • To identify individuals with a high risk of future cardiovascular disease • To initiate the appropriate medical management of newly diagnosed chronic diseases • To identify levels of potentially harmful drinking • To increase population level awareness of dementia specifically among 65 to 74 year olds • To work collaboratively with individuals whom require lifestyle modification and offer them on-going support through referral to one or more of the following local lifestyle intervention: o Livewell Lifestyle Service o National Diabetes Prevention Programme Service Providers/Clinicians delivering the Service must be a registered member of the UK/Irish General Medical Council (GMC) or Royal College of Nursing (RCN) and have a license to practise, if required by your regulator. 2 | Provision, fitting, monitoring, management, and removal of Subcutaneous Contraceptive Implants
This lot covers: • Fitting, monitoring, checking and removal of contraceptive implants in line with current guidelines on best practice (e.g. NICE guidance on LARC, Faculty of Sexual and Reproductive Healthcare) All implants must be licensed for use in the UK i.e. Nexplanon®. Fitting monitoring and removal should be in line with the most up to date Summary of Product Characteristics guidelines • Maintenance of an up-to-date practice contraception register including all patients fitted with a contraceptive implant. . • Sexual history taking. • Risk assessment. • Provision of condoms. • Patient information. • Consent. • Assessment and follow up. • Record Keeping. • Provision of adequate equipment. • Sterilisation and infection prevention and control. • Clinical skills and competencies. Service Providers/Clinicians delivering the Service must be a registered member of the UK/Irish General Medical Council (GMC) or Royal College of Nursing (RCN) and have a license to practise, if required by your regulator. Additional information: The initial Contract period is five years (5) the contract is estimated to commence 01 April 2022 and expire 31 March 2027. 3 | Provision, fitting, management, and removal of Intra-uterine Contraception Device Fittings (IUCD) and Intra-uterine System (IUS)
This Lot covers: • Fitting, monitoring, checking and removal of IUCDs/IUS as appropriate in line with current guidelines on best practice (e.g. NICE guidance on LARCs, Faculty of Sexual and Reproductive Healthcare) and manufacturers' recommendations. All IUCDs/IUS must be licensed for use in the UK., this includes the treatment and management of Menorrhagia in line with current guidance. An appropriately trained assistant who can monitor the condition of the woman and assist in an emergency should be present during insertion of IUC . Insertion of IUCD/IUS can take place any time during a menstrual cycle if reasonably certain the women is not pregnant or at risk of pregnancy . • Fitting of copper bearing IUDs for emergency contraception, within 5 days (120 hours) from the first unprotected sexual intercourse at any time in the cycle or up to 5 days after the expected date of ovulation in a regular cycle, provided that it is not contraindicated. • Removal of uterine device. • Maintenance of an up-to-date practice contraception register including all patients fitted with an IUCD. • Sexual history taking. • Risk assessment. • Provision of condoms. • Patient information. • Consent. • Assessment and follow up. • Record Keeping. • Provision of adequate equipment. • Sterilisation and infection prevention and control. • Clinical skills and competencies. • Prescribing. 4 | Primary Care Child and Family Weight Management Extended Brief Intervention Service
To deliver an extended brief intervention to a Parent/Carer of a child that's been identified as overweight or obese by the local National Child Weight Measurement Programme and in doing so motivate that adult such that they make food and activity choices that are positive for the family and the particular child. The ultimate goal is that the Parent/Carer actively reduces the child's weight using information and advice delivered as part of the brief intervention. The Service will offer: • An extended brief intervention by a trained professional (for example a nurse practitioner, social prescriber, healthy weight coach) to those parents/carers whose child has been identified as being overweight or obese (have a body mass index at or above the 91st centile) through the National Weight Management Programme. • The intervention will discuss a child's weight/growth, use behaviour change techniques to support acceptance and action, and offer tailored support and onward referral to tier 2 child weight management services or specialist services. • A follow up with the Parent/Career by a trained professional, to check on progress and to motivate further change within 3 months post the extended brief intervention. • Information on losing weight, healthy eating, increased physical activity and tier 2 local child management programmes to parents/carers who choose not to engage. Service Providers/Clinicians delivering the Service must be a registered member of the UK/Irish General Medical Council (GMC) or Royal College of Nursing (RCN) and have a license to practise, if required by your regulator. 5 | Pharmacy Based Needle Exchange
The following elements of Service delivery will broadly align to the objectives in the Specification. In delivering this Service the Service Provider will demonstrate compliance with all relevant national standards for service quality and clinical governance including compliance with their obligations under Schedule 4 of the Pharmaceutical Services Regulations (Terms of Service of NHS pharmacists) in respect of the provision of essential services and an acceptable system of clinical governance. The Pharmacy Based Needle Exchange Service will: • Offer a user friendly, non-judgemental, client centred and confidential service. • Provide needles, syringes and other equipment used to prepare and take drugs (e.g. swabs, filters, mixing containers) as supplied by the Derby City Council appointed distributor. • Provide individual waste bins for clients to safely dispose of and return used needle litter as supplied by the Derby City Council appointed distributor. • Provide advice on minimising the harms caused by illicit drug taking including advice on safer injecting practices, how to avoid and manage an overdose and the safe handling and disposal of injecting equipment. • Support clients to stop injecting drugs and switch to safer drug taking practices (if these are available) and encourage the client to access and engage with drug treatment. • In agreement with Commissioners and where need is appropriate, issue injecting equipment to those who take performance enhancing drugs and where this is not the case facilitate access and referral to specialist services for those with high risk injecting behaviours, including image- and performance- enhancing drugs. • Facilitate access to blood borne virus testing, vaccination and referral to treatment and where commissioned to do so deliver Hepatitis C testing to those not in treatment in accordance with the NHS Community pharmacy Hepatitis C Antibody Testing Service. • Facilitate access to other health and welfare services as appropriate. • Safeguard vulnerable adults and children Service Providers/Pharmacies delivering this Service must be registered with … Additional information: The initial Contract period is five years (5) the contract is estimated to commence 01 April 2022 and expire 31 March 2027. 6 | Community Oral Emergency Contraception Services
The Service Provider will supply Levonorgestrel Oral Emergency Contraception (OEC) when appropriate, to young women under the age of 25 years; in line with the requirements of the locally produced Patient Group Direction (PGD) (see appendix C), and provide support and advice to young women accessing the service, including advice on the avoidance of pregnancy and STIs, through safer sex and condom use, regular contraceptive methods, and provide onward signposting to Your Sexual Health Matters (Derbyshire Integrated Sexual Health Service (ISHS) website. Women over the age of 25 years will be appropriately signposted to alternative providers of free OEC and advised that they may purchase OEC at pharmacies over the counter at their own cost. Emergency contraception methods are not limited to OEC and include the use of Intra-uterine devices (IUDs). Though this Service only allows supply of OEC, it raises awareness of other methods of emergency contraception that are available and facilitates access to these. The healthcare practitioner(s) delivering this Service will be required to maintain a minimum of three OEC consultations per month in order to achieve the required quality standard. The Commissioner reserves the right to include the provision of ellaOne, within the scope of the Service delivery throughout the duration of the Contract, in order to align with population, need. 7 | Supervised Consumption
The Service will operate as part of the wider Drug and Alcohol Recovery Service currently operated from St Andrews House by Derbyshire Healthcare NHS Foundation Trust. Supervision of prescribed medication (methadone and buprenorphine) is essential in ensuring the efficacy of prescribed drug treatment - and in ensuring the appropriate safeguarding and risk management of those individuals. On accessing drug treatment, the prescribed medication forms part of a wider treatment plan. The Service Provider will support the delivery of the Service User's drug treatment plan by - • Assessing the Service User prior to dispensing prescribed medication to ensure they will not be put at risk of overdose (for example due to intoxication) by taking the medication • Dispensing prescribed medication in the specified instalments • Ensuring each supervised dose is correctly administered to the Service User for whom it was intended • Monitoring the Service Users' response to prescribed treatment and reporting any concerns to the Drug and Alcohol Recovery Service. • Monitoring the Service User's attendance at appointments and reporting any concerns to the Drug and Alcohol Recovery Service. • Liaison with others directly involved in care of the Service User (where the Service User has given their written consent) • Identifying and reporting any child or vulnerable adult safeguarding concerns • Reducing the risk of diversion of prescribed medicines onto the illicit drugs market • Avoiding accidental exposure of unused or unwanted medication.
