Description

NHS Norfolk & Waveney Clinical Commissioning Group (referred to as the Commissioner) is inviting suitably qualified and experienced providers to express their interest in delivering the NHS Continuing Healthcare Services in three lots under an Any Qualified Provider (AQP) framework, by responding to this opportunity as required and described in this document, and within the published tender. The contract will be awarded on a zero value, zero volume basis. This list will be akin to a Dynamic Purchasing System (DPS) and will allow providers to submit applications at any time throughout the life of the contract.

Lot Division

1 NHS CHC for Norfolk and Waveney - Nursing Homes
  • Value: £1

NHS Norfolk & Waveney CCG invite Nursing Homes across the NHS Norfolk and Waveney CCG footprint (and bordering areas), to join a Preferred Provider list for Continuing Healthcare services. The Service is for adults who are eligible for NHS Continuing Healthcare. Service Users placed using this Contract will have needs that may include but not be limited to End of Life, Learning Disabilities, Mental Health, Dementia, Physical Disabilities, Brain Injury, and Frailty. To meet the care needs of a wide range of people, the CCG encourages applications from Nursing Homes with a broad range of specialisms and services. Placements are more commonly long term but may include shorter term and respite arrangements plus rapid care for individuals approaching the end of their life. The contract length will be from the point of award until the expiration date of the contract (31st October 2024). The contract for the initial window will commence 1st November 2021. Applications throughout the life of the agreement and contracts will be awarded during the life of the contract if providers are successful. 2 NHS CHC Services for Norfolk and Waveney - Residential Homes

NHS Norfolk & Waveney CCG invite Residential Homes across the NHS Norfolk and Waveney CCG footprint (and bordering areas), to join a Preferred Provider list for Continuing Healthcare services. The Service is for adults who are eligible for NHS Continuing Healthcare. Service Users placed using this Contract will have needs that may include but not be limited to End of Life, Learning Disabilities, Mental Health, Dementia, Physical Disabilities, Brain Injury, and Frailty. To meet the care needs of a wide range of people, the CCG encourages applications from Residential Homes with a broad range of specialisms and services, but particularly those with specialisms in care for those living with Dementia or presenting with challenging behaviours. Placements are more commonly long term but may include shorter term and respite arrangements. The contract length will be from the point of award until the expiration date of the contract (31st October 2024). The contract for the initial window will commence 1st November 2021. 4 Lot No: 3 - NHS CHC Services for Norfolk and Waveney - Domiciliary Services

Norfolk & Waveney Domiciliary Care Provision for NHS Continuing Healthcare (NHS CHC) is for packages of care for adults who are eligible for NHS Continuing Healthcare. Individuals placed will have needs that may include but not be limited to End of Life, Learning Disabilities, Mental Health, Dementia, Physical Disabilities, Brain Injury, and Frailty The purpose of delivering NHS CHC within the domiciliary setting is to provide care at home for individuals with a primary health care need who are eligible for NHS CHC. This includes enabling individuals to achieve the best possible quality of life whatever their disease or disability, ethnic background, or sexual orientation. This will be achieved through the application of a holistic person-centered approach to care which is integrated seamlessly with all other health services, especially GP primary medical services, specialist palliative care services, mental health, learning disabilities and community social care. The contract length will be from the point of award until the expiration date of the contract (31st October 2024). The contract for the initial window will commence 1st November 2021. ** PREVIEW NOTICE, please check Find a Tender for full details. ** The AQP application process is open for the full term of the contract and never closes. Throughout the life of the contract, providers are able to submit an application to be part of the NHS Continuing Healthcare Residential & Domiciliary Care Preferred Provider list produced as an outcome of this procurement process. Once on the list the provider will then be able to accept patient admissions. Please note if a provider is unsuccessful with their application, they will be required to resubmit their application in its entirety and the whole submission would in turn be re-evaluated by the Commissioner. The contract length will be from the point of award until the expiration date of the contract (31st October 2024). The contract for the initial window will commence 1st November 2021.
