Description

Walsall Council is looking to commission one or two new providers to provide reablement in the Community/Discharge Home to Assess (DH2A) services on a short term basis. Reablement in the community or Discharge Home To Assess through Walsall's Intermediate Care Service (ICS) are in line with Home First principles which aim to enable people who are discharged from hospital to recover, reable or rehabilitate.
