Description

The Tier 3 Specialist Weight Management Service called 'The Weigh Ahead' is an autonomous service delivering the Tier 3 element of the wider weight management pathway and is commissioned collaboratively across 6 geographical areas within the NHS Hampshire, Southampton and Isle of Wight CCG, including the Isle of Wight area which will become a new geographical area served by the contract from 2021 onwards and the North East Hampshire & Farnham geographical area within the Frimley CCG North Hampshire North East Hampshire & Farnham South Eastern Hampshire Fareham and Gosport West Hampshire Southampton City Isle of Wight Spire Healthcare Limited is the provider of the service. The contract has been in place since 1st April 2017 and expired on 31st March 2020. The Contract is being awarded to Spire Healthcare for 3 years from 1st April 2021, with the option to extend for a further 2 year period to provide continuity of service delivery for local patients.

Total Quantity or Scope

As per detail contained in PIN 2021/S 000-017186 In line with the NHS Long Term Plan, providing a Specialist Weight Management Service within the community supports the new NHS model for the 21st Century in which patients get more options, better support and properly joined-up care at the right time in the optimal care setting. The NHS Long Term Plan also sets out new action that the NHS will strengthen its contribution to prevention and health inequalities to improve health; reduce health inequalities and reduce costs. The Hampshire and Isle of Wight Long Term Strategic Delivery Plan sets out how the local health and care community will deliver the ambitions and commitments set out in the NHS Long Term Plan and embed prevention of risk factors including smoking, alcohol, obesity and physical activity in all health and care pathways so that all patients will be offered referrals or signposted to appropriate preventative support. This includes the referral to the Tier 3 specialist service following the completion of the Tier 2. Hampshire, Southampton and Isle of Wight CCG have developed local place based plans to meet the needs of local populations. These plans build upon the national and regional plans.For example in Southampton, the Southampton City Health and Care Strategy 2020-2025,sets out the commitment to continue with the 'one city' place-based approach; working together to improve health and care outcome for the population of Southampton. It should be noted that not all patients starting the programme will complete the programme to generate a completers payment. However, there is the expectation that 80% of those who start the programme will complete. The schedule of indicative activity for each locality has been revised and the 2021/22 contract indicative activity plan is as follows. The volume of service users fluctuates, not all those who start the 6 month programme will complete it. The T3 service is a 6 month programme and therefore activity may extend across financial years.
