The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Suport Tool.

1 | CHC Nursing Homes - Award to Abbot Care Home The service is to deliver Continuing Healthcare Services to patients in Nursing Homes commissioned by West Essex Clinical Commissioning Group. The service supports people with long-term complex health needs who have been assessed, and qualify for, NHS funding. The service will be delivered against the local specification which is aligned to the 11 domains of the Decision Suport Tool. The service has developed the schedules within the NHS formal contract hat, when the contract is issued, will mitigate existing service risks by: - having a specific nursing home specification aligned to the 11 domains of the Decision Support Tool - An agreed rate for both nursing an ddementia beds for the contract year - Evidence based requests for one to one support - Provider quality dashboard reporting - Expedites placements, delivering faster acute discharge and improving flow - Enables a strong partnership to be developed with the provider The contract will be for a period of 12 months with the option to extend for a further 12 months. Legal Justification
The Continuing Healthcare Services within Nursing Homes across West Essex are currently operating with no formal contractual arrangements in place and have historically been spot purchased. This leads to a significant number of risks such as lack of provider due diligence, increased variation in pricing with increasing costs and inconsistency of services being delivered to our patient populations. West Essex CCG has been a partner with Essex County Council and the other Essex CCGs for a number of years to commission the Integrated Residential and Nursing Framework. This would enable a legally compliant route to market for these services. However, this has not been successful in West Essex with none of the homes within our area bidding to be approved onto the Framework. The Framework reopens every year and we are actively working with our local homes to encourage them to sign up to this Framework. Issuing longer term contracts to these suppliers on a short term basis will reduce the risk outlined above, and increase the quality of care provided to our patients through: - having in palce a specific nursing home specification which is aligned to the 11 domains of the Decision Support Tool - an agreed rate for both nursing and dementia beds for the contract term - evidence based requests for one-to-one support - introduction of provider quality dashboard reporting which will deliver assurance of care provided - expedited placements, delivering faster acute discharge and improving patient flow - enabling strong partnerships to be developed with our providers.
