Description
Contract for the provision of Accommodation and Support to citizens who are homeless at the point of discharge. This will include but not limited to:• Provide good quality accommodation that meets the needs of the citizens including those who are bariatric, and wheelchair users.• Provide flexible person-centred support ensuring that support is delivered in response to the client’s needs and allows them to maintain accommodation and improve their health and wellbeing outcomes.• Provide the lead and support which will assist clients to navigate and gain access to appropriate services, including but not limited to health care, substance misuse, education and/or support to gain employment.The Contract will last for 5 years (unless terminated under the terms and conditions of contract) with the provision to extend for up to an additional 2 years subject to requirements, funding and satisfactory performance.
Total Quantity or Scope
This will include but not limited to:• Provide good quality accommodation that meets the needs of the citizens including those who are bariatric, and wheelchair users.• Provide flexible person-centred support ensuring that support is delivered in response to the client’s needs and allows them to maintain accommodation and improve their health and wellbeing outcomes.• Provide the lead and support which will assist clients to navigate and gain access to appropriate services, including but not limited to health care, substance misuse, education and/or support to gain employment.• Follow the principles of the No Wrong Door Network to ensure that all service users are enabled to access the correct support by referring or enabling access (including getting clients to appointments) to other services as appropriate.• Provide support that promotes social integration and financial stability.• Regularly review the person-centred support agreement to up-date, record and monitor the progress of each client.• Developing the skills and capabilities of individuals and raise their aspirations, enabling them to be independent and self-sustaining. • Provide a service that is timely and available at the point of need.• Develop, maintain and coordinate effective working relationships with external agencies• Maintain and develop support knowledge and expertise.This will a Birmingham council only contract.
