North and Mid Hampshire ICP is seeking the views of Healthcare Providers in relation to a new Community Diabetes Service for North Mid Hampshire (covering Basingstoke, Winchester, Andover and their rural surroundings). The anticipated value of the Service is GBP £500,000 per annum.

F15516 - North and Mid Hampshire Community Diabetes Service North and Mid Hampshire ICP Request for Information (RFI) North and Mid Hampshire ICP is seeking the views of Healthcare Providers in relation to a new Community Diabetes Service North Mid Hampshire (covering Basingstoke, Winchester, Andover and their rural surroundings). In the North and Mid Hampshire area, there are approximately 21,000 people living with diabetes, which includes approximately 19,000 patients with Type 2 diabetes. From prevalence figures, we estimate that a further 10,000 people (based on 6.73% CCG prevalence), have yet to be diagnosed. The prevalence of diabetes is increasing at about 8% per annum in the under 75 age group. … This is a process designed to help the Commissioner form a view of the best way to commission the service and is not the beginning of a Tender exercise. Aims and objectives of the service are: • Educating and supporting healthcare professionals. • Advice and Guidance to GPs; • Interventions with patients, who are unable to be managed in Primary Care, using clinically effective therapies and management plans and sharing the therapy changes made or care plans with Primary Care; • Patient education and empowerment - providing various education programmes for patients with diabetes to promote healthy lifestyles and the capacity of patients to self-manage their condition • Health Care Professional education • Utilising the full range of Diabetes trained clinicians in an MDT format to prevent diabetes as well as monitor and treat diabetes patients • Effective cross organisational working and communication. (Primary Care, Secondary care, ICT teams, PCNs, community services, retinal screening services, Lifestyle services, Local Council, Third sector organisations, Patient support groups) Thank you - we appreciate your time and effort in completing this RFI.
