Description
Pluss has secured an extension of its ESF/National Lottery Community Fund (BBO) programmes in Devon, Cornwall, Somerset and West Yorkshire, until March 2023, and as such is aiming to extend the social prescribing element of the Cornwall projects. Pluss CIC is seeking one supplier to provide social prescribing software and support in the two Cornwall areas with the option of expanding into one other area (subject to funding). A successful pilot project has been in place in the two Cornwall projects for a year and PLUSS CIC now want to develop and extend this for the remaining year of the projects (extension into one other area will be subject to securing funding). The pilot project has enabled Pluss CIC social prescribers to refer participants to the most appropriate intervention for their health and wellbeing needs. It has also integrated with GP practices to enable more seamless referrals and work with the individuals who are judged to benefit from social prescription.
