Other Information
** PREVIEW NOTICE, please check Find a Tender for full details. ** ICS Intermediate Care Strategy This opportunity forms one part of a programme of work to develop a shared strategy and approach to short-term services for local people in need of recovery whether through admission avoidance or discharge to assess. A key priority for the ICS is to design and implement a more effective home and bed based offer through an integrated Intermediate Care offer. Keeping people well and supporting recovery at home remains our priority and through working more closely together, we wish to reduce the reliance on beds through the development of more sustainable services to support people at home. The long term vision for the model will be community health provider led step-down & up units that combine our intermediate care offer with the principles of recovery and proper wrap around support, and exploring opportunities for joint working. The model will aim to:- Recovery - Retain or regain skills, confidence and physical health to live the fullest life possible / maximise independence Enable & empower - Helping people make positive choices for themselves and their independence Assess - Helping access the right service but discussing eligibility or longer-term support once an immediate crisis is over. Things will look different because we will design: Wrap-around support, including closer working between social care, health, housing and other partners Home first - Proactively manage pathways so that when a person can receive support from home that is where they go More support in fewer settings Whilst we expect an increasing role for community health within bed based solutions, as an ICS we are committed to continuing to engage with residential and nursing providers about how we can work together to deliver effective admission avoidance and hospital discharge solutions now and in the future.
