Total Quantity or Scope
Stroke Association currently delivers post-acute stroke care in the Wakefield District and has built up a strong working relationship with the local therapy team, from whom it receives 200 referrals per annum. These 200 referrals represent the patient cohort that the Commissioner is specifically seeking to target to ensure that each is offered a 6-month review, post-stroke. Individuals within this cohort are most likely to require, and value, a 6-month review, as they are those receiving post-acute care to aid their recovery from a stroke.
