Description
NHS North West London ICB requires the provision of various Hillingdon Borough based Enhanced Primary Care Services to continue covering its registered population from 1st April 2023. This service can only be delivered by a provider with access to the PCN member practices registered list of patients. The contract comprises of the care home and care connection team services and are outcomes based that focus on improving clinical outcomes, driving improvements in the consistency of care provision, reduce unwarranted and reduce health and care inequalities for the population it serves. Services will benefit the PCNs GP member practices populations. NHS NW London ICB is of the view that The Confederation CIC, that is a legal entity in Hillingdon borough, is the only provider capable of delivering these services because the provider has access to the PCN member practices registered list of patients. This enables the provider to proactively identify patients and have access to the entire patient medical record. It also enables continuity of care and provision of services at a location closer to the patient's home. The purpose of this notice is to be transparent about NHS NW London ICB's intention to initiate a new contract with The Confederation CIC (NHS Standard Contract) for a period of one (1) year (estimated value £2,372,248 per annum). The ICB is committed to assessing the needs of the service users and considering how it should meet those needs, and improving the quality, efficiency and integration of services.
