Description

Staffordshire County Council (SCC) has a legal duty to ensure Children's Community Support (including Domiciliary Care) provision is in place to support Children and Young People (C/YP) with a range of Disabilities who have been assessed by a qualified Social Worker as being eligible for a personal care service within the family home and local community. This Service will comprise of four Lot's: • Lot 1- Lifeskills and Independence • Lot 2- Community and Home Care Support and socialisation • Lot 3- Sitting Services • Lot 4- Complex and Urgent Care

Lot 1- Lifeskills and Independence The provision of Lifeskills and Independence Short Breaks support must: • Provide a Short Break opportunity to parents/carers of Children/Young People (C/YP) with SEND. • Enable C/YP with SEND to work towards individualised, person-centred Outcomes with appropriate support to access their local community, preparing them effectively for adulthood. • Equip C/YP with SEND and their parents/carers with the tools and confidence to actively participate in their local community.

Lot 2- Community & Home Care Support and Socialisation The Service will focus on building resilience, maximising independence, following a strengths based, restorative approach. Providers will deliver a quality and effective Service personalised to each C/YP’s Assessed Need/s. The support from the Provider must contribute to enabling the family to live and feel supported in their family life with individualised Care/Support that reflects the C/YP’s needs. Providers will produce their own Care Plan and individual Risk Assessments personalised to the C/YP. The partnership between the family and the Provider will aim to maximise the potential of each C/YP to assist with daily routines and support the parent/carer to be able to successfully maintain their primary caring role. The focus of the Service is to promote independence, social inclusion, safe care, positive risk taking, and general wellbeing of the C/YP and their family. The Service will support this through the following objectives: • Consistently promoting independence through an enabling approach at every possible opportunity. • Achieving the Outcomes in the C/YP’s Care Plan. • Involving families to be part of the Assessment and Service delivery process. • Encouraging self-care and ‘wellness’. • Arranging access to specialist support when needed. • Focussing support to help C/YP to remain in the home environment. • Motivating the C/YP to maintain or develop their life skills. • Supporting the family to engage with their wider community

Lot 3 – Sitting Service This Service is to ensure parents/carers are supported to take a short break from their caring role and responsibilities when caring for C/YP age 0-18 who have SEND. The Provider must offer flexible sitting services that can be provided during the day, evening, or night for any length of time, as follows: • C/YP can be supported in the home, giving the carer time to go out if they choose. • C/YP may wish to go out in their local community for the allocated hours to pursue their own interests. • Support workers can accompany parents/carers and the C/YP on outings to provide the extra support required for the whole family unit.

Lot 4 – Complex and Urgent Care Providers will be expected to deliver Care/Support in an urgent situation/emergency, within the home, to ensure the C/YP Social Care Needs are met. Support will be individualised to the C/YP needs, enabling the family to stay safe and well together, deescalating the risk of the C/YP entering the local authority care system. This support could be surrounding complex care needs, Challenging Behaviour, and life limiting conditions. The Provider may need to work alongside healthcare and other professionals to deliver care and support. The Provider must comply with all Relevant Legislation. The Service will be provided by appropriately qualified Staff, in line with the standards set out by the CQC.
