Description
Bristol, North Somerset and South Gloucestershire Integrated Care Board ("BNSSG ICB") is seeking to commission a provider to carry out Continuing Healthcare ('CHC') assessments. This service will supplement the assessment activity undertaken by the ICB's Funded Care Team, ensuring that individuals requiring a full CHC assessment in BNSSG will wait no longer than 28 days from referral to decision. The successful provider will comply with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (2022 revised). The successful provider will be expected to: • Undertake the lead CHC assessment role for a defined cohort of CHC assessments. • Assess and record robust information and evidence the current level of need individuals in the BNSSG Care Needs Summary. • Review available evidence and apply information to the DST. • Demonstrate proactive engagement with individuals being assessed for CHC, their families and/or representatives. • Ensure effective engagement at all stages with key stakeholders. • Work in collaboration will local authority colleagues to promote participation in CHC assessments. • To book and coordinate assessments. • Evidence and record recommendations to Caretrack. • Report KPI achievements weekly. • Ensure all service users referred by the ICB are appropriately assessed in a timely manner, in line with expectations set out in this specification. • Act as the 'ICB representative' at MDT meetings. • Inform the ICB where an advocate is required to support an individual through the eligibility process. • Keep the ICB informed where a delay occurs or is anticipated via the agreed Reporting Framework mechanisms. • The ICB are provided with a weekly Programme tracker report of progress, to include query management and Programme Anomalies. • Virtual assessments are anticipated from the provider, any requests for face-to-face assessment are to be returned to the ICB to facilitate. Further details of requirements can be found in the specification. The contract will be for 6 months with an estimated budget of £160,000 exclusive of VAT. This procurement is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.
