Care at Home Services for older people and adults with physical disabilities Provision of a Care at Home service for adults with physical disabilities (age 16+) and older people (age 65+). Care at Home for those whose primary needs are mental health or learning disabilities specific support are not in scope of this contract. Replacement Care for the purpose of a Short Break for the Carer is not in scope of this contract. Live-in services are not in the scope of this contract.
