Description

We are seeking an experienced weight management provider that can demonstrate depth of understanding of the complexity of overweight and obesity and detailed knowledge of behaviour change theory. Some aspects of this specification deviate from the standard 12-week programme approach to weight management. There are also specific aims regarding engagement of priority population groups. For this reason, the first 2 years of the contract will focus on developing and implementing delivery models During this time the provider will work in partnership with the commissioner and local stakeholders to develop and implement the local delivery models for both the adult and family service. Data will be collected to understand uptake and impact across different population groups (minimum dataset). This information will then inform further development and refinement of the local delivery model on an ongoing basis. A formal review point is built in at the end of Year 2.

Total Quantity or Scope

The overall contract aim is to support adults, children and young people to: • Develop realistic, healthy and sustainable habits in relation to food, physical activity and general well-being. • Maintain or lose weight. The service has four core objectives 1. Deliver a service that supports adults to adopt healthier behaviours and to set and achieve personalised and realistic goals regarding their weight and overall health (As per the specification for the Holistic Adult Weight Management Service in section 4 which includes sub-contracting responsibilities). 2. Temporarily support any remaining service users on the adult Tier 2+ pathway (As per section 5) 3. Deliver a service that supports children, young people and their families to adopt healthier behaviours and to set and achieve personalised and realistic goals regarding their weight and overall health (As per the specification for the Holistic Family Weight Management Service in section 6). This includes delivering a family weight management service, pro-active feedback calls to families with child above the 99.6th centile and NCMP linked proactive work in a small selection of schools. 4. Market the adult and family services to referrers and residents across the city including through the provision of brief intervention training that supports effective, supportive and compassionate conversations about weight.
