Description
Tower Hamlets intend to commission a floating support service that offers support to residents within their own home or a setting of their choosing to sustain their mental wellbeing and recovery through the delivery of person-centred support. The service will take a strengths-based approach to support delivery, capitalising on residents existing capabilities to support them to sustain their recovery and continue to live independently, thus preventing from accessing more institutional forms of care and hospital admission. The provider will offer regular 'drop in' sessions at a suitable setting within the borough. Residents attending drop in sessions may be existing service users or those that have previously used the ILCS service and require support short term in order to maintain their wellbeing and/ or recovery. Whilst the majority of staff time will be utilised visiting service users, the successful provider will be required to source a suitable space within the boundaries of Tower Hamlets to enable staff desk-based working from the contract start date. This space must include a private space which can be used for meetings with service users and 'drop in' sessions. The provider will support the coordination of the borough's category band 1b social housing quota used to accommodate those exiting the borough's mental health supported living pathway. The provider will deliver awareness and learning sessions to residents within the supported living pathway, liaise with partners to progress housing applications and provide transitional support for a minimum period of 6 months for those exiting the supported living pathway into their independent accommodation. The provider will continue to support historic housing management functions for those within Bishops Way. The successful provider will support the council to achieve its priorities and demonstrate a commitment to support the local economy and employment of local people through the lifetime of the contract.
