Description

South Yorkshire's Integrated Care Board (ICB), Doncaster Place have commissioned an independent evaluation of the existing service offer which is in place for those with an SMI to access annual PHCs in Doncaster. You are invited to a Market Engagement Event where the evaluation will be reviewed and options around potential future service models will be discussed. The current service was procured in 2019 and is provided by Primary Care Doncaster, sub-contracted to RDaSH. The contract is due to expire on the 15th June 2024. Currently on the General Practice SMI register there are 2,443 patients on the list (excluding patients recorded as 'in remission'.) The service offers a holistic physical and mental health check inclusive of the following 6 elements of the core PHC: 1. A measurement of weight (BMI or BMI + Waist circumference) 2. A blood pressure and pulse check (diastolic and systolic blood pressure recording + pulse rate) 3. A blood lipid including cholesterol test (cholesterol measurement or QRISK measurement) 4. A blood glucose test (blood glucose or HbA1c measurement) 5. An assessment of alcohol consumption 6. An assessment of smoking status The current service also offers the following checks: 1. An assessment of nutritional status, diet, and level of physical activity (nutrition/diet status + physical activity/exercise) status 2. An assessment of use of illicit substance/non prescribed drugs (substance misuse status) 3. Medicines reconciliation and review

The independent evaluation, led by Health Watch Doncaster will provide a number of potential options / considerations in terms of future service provision. SY ICB Doncaster Place will hold a market engagement event with interested providers and relevant stakeholders to review the evaluation and discuss potential future options which will support in the service specification development. Following the event, if the decision to go out to formal procurement is made, this will be published on the Atamis Procurement portal between December 2023 and January 2024. The financial envelope will also be published at this time.
