Description
The Glasgow City Health and Social Care Partnership is seeking dynamic organisations or partnerships of organisations to deliver community led health and wellbeing programmes that will be a key component of its Primary Care Improvement Plan and complement its strategic direction for health improvement and the work of its own staff and many partners via individual and group support in GP practices/clusters/primary care settings through the Community Link Worker programme.
Lot Division
1 | North West Community Link Workers will be located within primary care settings as part of a GP practice or cluster. Work will flow from practice(s) however the Community Link Worker will also provide support to clusters they are attached to. The programme will work mainly with adults aged 16 and over and their families in areas of deprivation, along with the population groups who experience significant social and health inequalities. The programmes will have the fundamental aim of providing support and building capacity at both individual and community levels. Programmes may be focused at the levels of GP practices/clusters, geographic communities or targeted at specific population groups. |

7 | South Community Link Workers will be located within primary care settings as part of a GP practice or cluster. 9 | CAHMS This lot is for a thematic CLW post(s) which will be embedded in our Intensive Child and Adolescent Mental Health Service (CAMHS) team, and potentially support the unscheduled CAMHS team and the Eating Disorders (ED) Connect Team. These services offer Tier four support to Children and Young People and their families/carers. The Intensive CAMHS team work with young people requiring intensive support for a range of mental health issues including but not limited to Obsessive Compulsive Disorder, those at risk of suicide, eating disorders. The Intensive CAMHS team aims to prevent hospital admissions, and also support children and young people being discharged from hospital and offer intensive therapeutic supports to supplement care from Tier 3 CAMHS. |
