Description

The Provider will provide care and/or support to Individuals in their homes, including buildings with communal areas in which Individuals have tenancies as part of a Supported Living service (the Supported Living property). Where included in an Individual’s care and support plan this may include supporting Individuals to carry out and/or arrange for domestic tasks to be undertaken.

Total Quantity or Scope

This Service will be provided to Individuals who, unless otherwise agreed: • Are aged 18 years and over and • Are Ordinarily Resident in Somerset and • Have been assessed as having care and/or support needs that meet the National Minimum Eligibility Threshold under the Care Act (2014). • OR, individuals who meet the Somerset Council criteria for funded support.

Renewal Options

Extensions to the contract are available up to a maximum of 60 months.
