Please note we are currently only seeking expressions of interest and to inform you of the opportunity for market engagement. As such, no documentation is currently available. We wish to carry out engagement with the market prior to release of a contract notice. If you wish to engage with us, please express an interest in this project (after registering on intend). Norfolk County Council's Public Health team are seeking to procure a suitable Provider to deliver a Norfolk wide NHS Health Check (NHS HC) community provision service. The service will comprise of the following key elements: o Flexible and accessible community-based provision delivered by trained practitioners as per the NHS HC Competency Framework. o Minimum target delivery of 22,700 NHS HCs. o A focus on engaging innovatively with communities across Norfolk, as well as to targeted groups where there are higher levels of deprivation and health inequalities. o Working closely with GP practices across Norfolk to provide capacity support, raise awareness of NHS HCs and encourage NHS HC invitations. o Providing support with the expected roll-out of the NHS Digital Health Check in 2024. Indications at this stage is of a potential minimum lifetime contract value of ?1.89 million (non-capped) over 3 years (3-year possible extension), delivered as a block contract of ?420,000 per year. Start of contract 1st October 2024.
