Description
Framework agreement for CQC registered care homes and care homes with nursing located in the borough of Stockton-on-Tees. This tender is limited to Care Home Settings located within the boundaries of Stockton-on-Tees Local authority area that have a CQC registration with CQC for the delivery of Residential Personal Care Services and/or Nursing Care Services.
Lot Division
1 | Residential Care (permanent, short stay, respite or assessment and discharge support The provision of accommodation for persons who are aged over 18 and require personal care. Residential Care Services, based on a recognised assessed care need, in line with the individual's Care and Support Plan and the Service Specification included in this tender. |
2 | Residential Care - Dementia (permanent, short stay or respite): The provision of accommodation for persons who are aged over 18 and require personal care with assessed additional needs. Residential Care Services, based on a recognised assessed care need, in line with the individual's Care and Support Plan and the Service Specification included in this tender. |
3 | Nursing Care (permanent, short stay, respite or and discharge support) The provision of accommodation for persons who are aged over 18 and require nursing care. Residential Care Services, based on a recognised assessed care need, in line with the individual's Care and Support Plan and the Service Specification included in this tender. |
4 | Nursing Care - Dementia (permanent, short stay or respite) The provision of accommodation for persons who are aged over 18 and require nursing care. Residential Care Services, based on a recognised assessed care need, in line with the individual's Care and Support Plan and the Service Specification included in this tender. |
5 | Short-term Interim Rehabilitation Beds Short term (up to 6 weeks) funded placement to support rehabilitation to regain independence with an aim to return to their own homes as soon as possible. These beds are part of the winter resilience plan. Residential Care Services, based on a recognised assessed care need, in line with the individual's Care and Support Plan and the Service Specification included in this tender. |
6 | Bariatric care Services within adult bariatric population with a physical disability but should also be made available to other adults where their short-term needs can be facilitated within these settings and there is supply available. Residential Care Services, based on a recognised assessed care need, in line with the individual's Care and Support Plan and the Service Specification included in this tender. |
7 | Designated area (e.g. wing or unit) reserved for adults of the same gender Residential Care Services, based on a recognised assessed care need, in line with the individual's Care and Support Plan and the Service Specification included in this tender. This Lot would be subject to further competition that would indicate the type of provision required. Any provider who has indicated that they are able to offer this accommodation would be approached. |
