End of life bedded and home care support. Regulated Community Care and discharge Capacity (adults). Delivery of individualised and personalised care planning, individual support in the community/at home, discharge capacity. This is a Provider Selection Regime (PSR) confirmation of contract award notice. This contract has been awarded under the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award NHS Provider Selection Regime Direct Award B.

The current spec has been reviewed and there are no significant amendments. Decision makers are: Deputy Director of Quality, Head of CHC, Head of Nursing, Contract Manager, Finance Manager, Procurement manager No conflicts of interest registered Contract value - £2,206,000 over lifetime of contract 2 years
