Description
Due to a change in e-tendering system utilised by the Contracting Authority, this notice is being published to maintain the principles of the original process and enable providers, who have not already been successfully accredited and awarded a contract following the publication of the initial notice in August 2021 (FTS Ref: 2021/S 000-019005, https://www.find-tender.service.gov.uk/Notice/019005-2021), to express an interest and submit an application.Providers who already hold a contract relating to this service do not need to re-submit / re-apply.This Procurement is being advertised in accordance with the requirements of the Public Contracts Regulations 2015 (as amended) (the “Regulations”) to the extent that the Regulations relate to services within the scope of Schedule 3 thereto (Social and Other Specific Services).

1 | CHC Nursing Homes
NHS Norfolk & Waveney ICB invite Nursing Homes across the NHS Norfolk and Waveney ICB footprint (and bordering areas), to join a Preferred Provider list for Continuing Healthcare services. The Service is for adults who are eligible for NHS Continuing Healthcare. Service Users placed using this Contract will have needs that may include but not be limited to End of Life, Learning Disabilities, Mental Health, Dementia, Physical Disabilities, Brain Injury, and Frailty. To meet the care needs of a wide range of people, the ICB encourages applications from Nursing Homes with a broad range of specialisms and services. Placements are more commonly long term but may include shorter term and respite arrangements plus rapid care for individuals approaching the end of their life.Contracts awarded following publication of the initial notice/window relating to this service were for a maximum period of 3 years with an option to extend for a further 2 years, commencing on 1st November 2021.Contracts awarded after the 1st November 2021 will be from the point of award until the 31st October 2024, with the option to extend for a further 24 months subject to satisfactory performance. |

Norfolk & Waveney Domiciliary Care Provision for NHS Continuing Healthcare (NHS CHC) is for packages of care for adults who are eligible for NHS Continuing Healthcare. Individuals placed will have needs that may include but not be limited to End of Life, Learning Disabilities, Mental Health, Dementia, Physical Disabilities, Brain Injury, and Frailty The purpose of delivering NHS CHC within the domiciliary setting is to provide care at home for individuals with a primary health care need who are eligible for NHS CHC. This includes enabling individuals to achieve the best possible quality of life whatever their disease or disability, ethnic background, or sexual orientation. This will be achieved through the application of a holistic person-centered approach to care which is integrated seamlessly with all other health services, especially GP primary medical services, specialist palliative care services, mental health, learning disabilities and community social care.Contracts awarded following publication of the initial notice/window relating to this service were for a maximum period of 3 years with an option to extend for a further 2 years, commencing on 1st November 2021.Contracts awarded after the 1st November 2021 will be from the point of award until the 31st October 2024, with the option to extend for a further 24 months subject to satisfactory performance. |

This opportunity will allow providers to submit applications at any time throughout the life of the contract. The Accreditation Qualification Questionnaire Process (AQQ) contracts are awarded on a zero value, zero volume basis.The total available contract will be for a period of 5 years with an option to extend for up to a further 2 years at the discretion of the commissioner. However the contract length will be from the point of award until the expiration date of the contract be it 31st October 2024 for the initial period or 31st October 2026 if the full extension option is invoked. The contract for the initial window commenced 01st November 2021.The contract dates (01st September 2024 - 31st October 2026) in the "dates" fields are incorrect. Applications can be submitted and will be evaluated throughout the life of the agreement and contracts will be awarded during the life of the contract if providers are successful.
