Tunbridge Wells Borough Council (“the Council”) is conducting a tender exercise with and on behalf of its partner authorities, Sevenoaks District Council and Tonbridge and Malling Borough Council, to award a number of contracts for the provision of Hospital Discharge, Home Straight and Handyperson services. These include: Lot 1. Hospital Discharge Service – All partners Lot 2. Handyperson Service – Tunbridge Wells Borough Council only Lot 3. Handyperson Service – Tonbridge and Malling Borough Council only Lot 4. Home Straight Co-ordinator Service – Tunbridge Wells Borough Council only Lot 5. Home Straight Co-ordinator Service – Sevenoaks District Council only Lot 6. Better Care Fund Co-ordinator - Sevenoaks District Council only
Lot Division
1 | Lot 1. Hospital Discharge Service – All partners The purpose of the service is to provide practical support to help patients with a housing related issue to leave hospital and go home in a safe and timely manner, and to deliver interventions to prevent hospital admission or readmission. The service works closely with NHS colleagues within the hospital discharge teams at Maidstone and Tunbridge Wells NHS Trust. The service is open to anyone who is being discharged from hospital and has a housing related issue, or who is at risk of an unplanned admission or re-admission, where there is an unresolved housing issue affecting their wellbeing or has the potential to cause a stay in hospital. |
2 | Lot 2. Handyperson Service – Tunbridge Wells Borough Council only The purpose of the role is to provide a minor household repairs and small works service to a high standard, respecting the customer’s privacy and their property. The role will work alongside the Hospital Discharge service (see Lot 1) and the Home Straight service (see Lot 4). |
3 | Lot 3. Handyperson Service – Tonbridge and Malling Borough Council only The purpose of the role is to provide a minor household repairs and small works service to a high standard, respecting the customer’s privacy and their property. The role will work alongside the Hospital Discharge service (see Lot 1) and the Home Straight services (see Lots 4 and 5). |
4 | Lot 4. Home Straight Co-ordinator Service – Tunbridge Wells Borough Council only The purpose of the role is to work with and engage the customer to work towards de-cluttering their home, working with partner agencies and services, and achieve long term change, respecting customer individuality and maintain dignity and confidentiality at all times. |
5 | Lot 5. Home Straight Co-ordinator Service – Sevenoaks District Council only The purpose of the role is to work with and engage the customer to work towards de-cluttering their home, working with partner agencies and services, and achieve long term change, respecting customer individuality and maintain dignity and confidentiality at all times. |
6 | Lot 6. Better Care Fund Co-ordinator - Sevenoaks District Council only The purpose of this role is to provide support to the Council’s Private Sector Housing with the administration of disabled facility grants including carrying out home visits. In addition, the role will liaise with Health and Housing Coordinators based within three acute sites and provide support across any other initiatives funded by the Better Care Fund. |
