Description

The services the provider shall provide, will comprise of personal care, support with activities of daily living, social activities and nursing care where registered and required, specialist care and respite. The services shall include Residential Care services and / or Nursing Care services to the extent specified in the Contract Letter. The services will be procured under 5 lots: • Lot 1: Long Term Placements In Borough; • Lot 2 Long Term Placements Out of Borough; • Lot 3: Discharge to Access; • Lot 4: Respite Care; • Lot 5: Innovation and Social Enterprises

Lot Division

1 Long Term Placements In Borough
  • Value: £216M

Long term placements within Trafford. The services shall be provided to all persons, including people who: a) are at end of life; b) have a learning disability; c) have mental health issues; d) have dementia; and / or cognitive impairment e) have a physical and / or sensory disability. The services do not include provision of: a) FNC / CHC services which are statutory requirements of the NHS;; b) any services to persons under the age of 18; or c) Services to any person detained under the Mental Health Act 1983 with the exception of those whom the Council has guardianship over.

2 Long Term Placements Out of Borough
  • Value: £216M

Long term placements outside of the Trafford borough. The services shall be provided to all persons, including people who: a) are at end of life; b) have a learning disability; c) have mental health issues; d) have dementia; and / or cognitive impairment e) have a physical and / or sensory disability. 3 Discharge to Access

Services that provide short-term care to people who are clinically optimised and do not require an acute hospital bed, but may still require care services in the most appropriate setting to enable a full assessment of needs to be competed.

4 Respite Care
  • Value: £216M

Services that provide short-term care to a person, which temporarily replaces their exiting care to provide respite for both the person and the care giver.

5 Innovation and Social Enterprises
  • Value: £216M

Services that are innovative which promote market shaping and sustainability to deliver high quality care that meets the needs and requirements of older people.
