Description

Hospital Discharge The Hospital Discharge service will support people aged 60 years+ who are being discharged from Hospital without a formal care package and without the assistance of friends or family. The aim of the scheme is to support people up to the first 3 weeks after discharge to prevent readmission to hospital. The package offered by the Hospital Discharge Service is designed to ensure older people can remain independent in their own home, instead of taking away the skills required for independent living the team are proactive in encouraging and supporting those skills are maintained. Living Well Link The purpose of the Living Well Link Service is to put older people in personal control of their health and well-being and provide them with integrated care and support in their local community, focusing on prevention of ill health and promotion of well-being. This social prescribing service allows older people to be both healthy and independent whilst reducing the burden on NHS services in Sunderland through a reduction in need for GP visits, Out of Hours Contacts, Emergency admissions etc. Essence The Essence Service (the Service) provided by Age UK Sunderland (the Provider) will bring together patients with a formal, medical diagnosis of dementia in the early and early onset stages to create a supportive environment for them to retain their confidence, personal interests and wellbeing. The service is titled 'Essence' as it aims to keep the Essence of the individual focussing on the patient, rather than their dementia diagnosis and look to nurture and maintain their sense of individuality whilst responding to their developing dementia needs. The needs of carer will be considered both in relation to their caring responsibility and their own personal needs. Social Focus The project offers a chance for people aged 50+ with mild to moderate depression, anxiety or stress to get together & participate in various activities. The aim of the service is to improve their quality of life and help them maintain independence and reduce feelings of depression and anxiety etc. The intention is to award a contract to the existing provider via Direct Award Process C from 01 April 2024 - 31 March 2025. The final contract value is still to be finalised as it is dependent on national NHS planning guidance which is still to be released. As an indication the 2023/24 contract value was £657,830
