Description
In April 2013, the Health and Social Care Act 2012 set out Local Authorities statutory responsibilities for commissioning the NHS Health Check programme. Cornwall Council are inviting all GP Surgeries within Cornwall and the Isles of Scilly to join a light touch, Approved Provider List for the Provision of Health Checks. Service Users registered with the Service Provider who meet the criteria (Service Specification 2.2.1) should be invited to attend a face-to-face NHS Health Check once every five years. The "Invitation and Offer" can be sent by letter, email, or SMS. Service Providers can also offer opportunistic appointments. The "Invitation and Offer" should meet the Accessible Information Standard. The eligibility criteria are that the Service User must be registered with a GP Service Provider within Cornwall and Isles of Scilly and consequently all Providers looking to join the Approved Provider List will have to be a registered GP within Cornwall and Isles of Scilly.
Total Quantity or Scope
The NHS Health Check programme aims to improve the health and wellbeing of adults aged 40-74 years through the promotion of early awareness, assessment, and management of the major risk factors for cardiovascular disease – risk factors that are associated with premature death, disability and health inequalities of residents in Cornwall and Isles of Scilly. The objectives of the programme are: 1 To promote and improve the early identification and management of the Service Users’ behavioural and physiological risk factors for vascular disease and the other conditions associated with those risk factors 2 To support Service Users to effectively manage and reduce behavioural risks and associated conditions through information, behavioural and evidence based clinical interventions 3 To help reduce inequalities in the distribution and burden of behavioural risks, related conditions and multiple morbidities. Cornwall Council is utilising the light touch regime to produce an Approved Provider List for a five year period for all GP's registered in Cornwall and the Isles of Scilly to provide this service via a pathway to all eligible residents: The service pathway is made up of four key components below: • Invitation and Offer • Risk Assessment • Communication of Risk • Risk Management
