The delivery of annual physical health checks for patients with Severe Mental Illness (SMI) provided by GP Practices across Nottingham and Nottinghamshire. A contract award has been made following the Provider Selection Regime direct award process A. The lifetime value of the contract is £276,000 The dates between which the service will be contracted for are 1 April 2024 to 31 March 2026.
