Description

Provide a specialist community stop smoking service for people with Severe and Enduring Mental Illness (SMI) or a Common Mental Health Illness (CMHI) that complements their existing in-patient service.

Total Quantity or Scope

This service will provide a specialist community stop smoking service for people with Severe and Enduring Mental Illness (SMI) or a Common Mental Health Illness (CMHI) that complements their existing in-patient service. This would include following up with patients on discharge who have begun their programme in hospital. In addition, The provider would host a new co-ordinator role to work across the Leeds Integrated Tobacco Dependence Treatment Service collaborative to manage shared functions across the collaborative including data sharing, collection and submission, communications, stakeholder engagement, monitoring, linking with contract managers to support service improvement, supporting the development of pathways and training, and help with the mobilisation of the new activity being funded by this grant. This includes the purchase of a licence for Quit Manager, a case management system that can be used by all the services to make data collection and reporting easier.
