Description
Liverpool City Council intends to appoint a provider who will ensure the delivery of its future 0 to 19 (25) Public Health Nursing Service to deliver the Healthy Child Programme (HCP) in Liverpool. Transitional support will be available for young people, up to 25 years, with special educational needs and disabilities (SEND) and for those with experience of the care system. The service will be provided by a skill mixed team led by Specialist Public Health Nurses (Health Visitors and School Nurses).
Total Quantity or Scope
The new service will provide an innovative, integrated model of delivery that focuses upon the Council’s new neighbourhood structure. The service will build upon the ethos of the Family Hub programme and support a platform for system change across all partners in the city. The service will be based on a whole family, life course model to facilitate a greater degree of integration of services to maximise efficiency and coordination. The service will be underpinned by the principles of accessibility, connectivity, and relational practice.
