This procurement will identify a Bidder who will provide a Mental Health Community Support Service (Hammersmith & Fulham) for NHS North West London Integrated Care Board. The provision of a mental health community support service in Hammersmith and Fulham is required for individuals aged 18-65 with mental health problems. The service targets adult’s mental health needs, offering a range of interventions to support individuals transitioning from Mental Health Integrated Network Teams (MINT) and preventing the need for secondary care services.
