Contract Details: Final decision by the decision makers, Stockport NHS FT (the FT) to award the contract to: Marple Cottage Surgery, 50 Church St, Marple, Stockport SK6 6BW. The contract will run from 1st October 2024 up to 30th September 2026. Reasons for selecting existing supplier against the Key Criteria: Quality and Innovation - For a number of years the supplier has been providing safe and efficient Primary Care medical services for patients with No Criteria to Reside who require care and support, at the Discharge to Assess (D2A) facility managed by the Trust, at The Meadows. A GP lead ensures ongoing care plans are in place for patients; that appropriate medication is prescribed and available and that issues and concerns are raised at contract meetings and escalated prior to this where appropriate. The supplier has a proven record of working well with the FT to provide the service. Integration, collaboration, and service sustainability - The supplier has worked collaboratively with the FT to develop local processes and procedures to ensure that a safe and effective insourced service is provided on-site at the Meadows. The supplier has an identified GP lead but has allocated sessions from salaried GPs to cover this work, back filling general practice work, to ensure continuity of care. Improving access, reducing health inequalities and facilitating choice - As well as providing daily ward rounds (Mon - Fri) and next day assessments of newly admitted patients the supplier attends weekly MDT meetings, provide timely discharge summaries (within 24hrs) and contribute to maintaining an average length of stay in the unit of
