Description
Hertfordshire County Council (the Council) has the requirement for the provision of Admiral Nurses to support and work in conjunction with the Community Dementia Support Services. The service provider will host qualified specialist dementia nurses who work in partnership with people affected by dementia and their families by embracing evidence-based relationship-centred care.
Total Quantity or Scope
Hertfordshire County Council (the Council) has the requirement to provide an Admiral Nursing service as part of the wider Community Dementia Support Services. The Service is, and will be, led by qualified specialist dementia nurses who work in partnership with people affected by dementia and their families by embracing evidence-based relationship-centred care. They facilitate choice through providing a range of expert clinical, practical and emotional support through the whole pathway and through difficult transitions, building resilience and empowering family carers by encouraging problem-solving approaches thus addressing any potential access or inequality barriers. The role of the Community Dementia Support Services (under which this Admiral Nursing Service also sits) within the Adult Care Services (ACS) vision is about what the VCFSE (Voluntary, Community, Faith & Social Enterprise sector) can do in terms of prevention - part of the connect, prevent and enable part of our Connected Lives principles, as well as the Think Local Act Personal (TLAP) approach, developed to improve the quality of life for people who use social care services. The Council is intending to award this Contract to our existing provider following Direct Award Process C under the Provider Selection Regime (PSR) The Contract will commence on the 1st October 2024 and will be for a three year and six months (3.5 year) core period with the option to extend for up to a further two (2) years. The approximate life-time value of this Contract is as stated in II.1.7 above and includes the value of any possible extensions.
