Description
This VEAT is to inform the market of the ICB's intention to direct award a contract to Age UK, whilst a procurement process is planned to test the market. The Take Home and Settle Service (THAS) safely support patients' discharge from hospital. Short term practical support is provided in the person's home or other interim accommodation to support independence and prevent readmission to hospital. The service enables individuals to regain confidence, motivation and skills to self-manage, and encourages them to engage with the local community to improve socialisation and wellbeing.
Total Quantity or Scope
The Take Home and Settle Service (THAS) is to safely support patients' discharge from hospital. Short term practical support is provided in the person's home or other interim accommodation to support independence and prevent readmission to hospital. The service enables individuals to regain confidence, motivation and skills to self-manage, and encourages them to engage with the local community to improve socialisation and wellbeing. It provides a rapid response service (within 30 minutes of receipt of a call) 7 days a week (including public holidays) Monday to Friday 09.00-21.30 and weekends 09.00-17.00, to prevent delayed discharge and provide safe discharge from the Queen Elizabeth Hospital Woolwich. The service is for people over the age of 18 who, after treatment, do not need to be admitted to hospital, or who can be safely discharged from hospital. The service assists in enabling safe discharge and preventing readmission following discharge by undertaking pre-discharge checks where appropriate, and by accompanying people home and staying with them for a short period to ensure that they settle safely back into their home environment. They also ensure that patients have information on how to access services and activities.

Legal Justification
