Description

Stroke recovery services seek to provide guidance to patients and their families on how to live healthy lives after suffering a stroke and hospital admission. The service signposts patients on how to receive onward care and access community assets to minimise the occurrence of future stroke episodes. The service provides advocacy where required but is generally targeted towards providing personalised accessible information and tools and empower people to self-manage their care.

NHS Cheshire and Merseyside ICB - intend to award a 1 year contract via Direct Award C incumbent provider to deliver Community Stroke Recovery and Support Services in Cheshire & Merseyside (excluding St Helens Place) .
