Description
Fortnightly face to face clinic at Northside House for 1 session (either always a GP, or a monthly GP clinic and a monthly Nurse Practitioner clinic who could alternate to provide 1 session per fortnight). This is a secure mental health unit, and the contract will run over a 12 month period. Primary care review/advice for chronic disease management which will include, but not limited to, as below: • Primary care asthma care & reviews from trained practitioners • Primary care COPD care & reviews from trained practitioners • Primary care diabetes care & reviews from trained practitioners - to support in-patient staff.

This contract award has been done so in accordance with the 'Urgent' criteria of the Provider Selection Regime guidance. The urgent contract award is to ensure continuity of service, following the previous service provider entering administration. The urgent award of contract is to ensure continued provision of service to Trust patients.
