Description
AQP's delivering NHS Health Checks face to face across Derbyshire to the eligible population who meets the NHS Health Check criteria. This is a Provider Selection Regime (PSR) confirmation of contract award notice. This contract has been awarded under the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. This contract is awarded as per the award criteria of Direct Award 'B' under the PSR regulations. The contact date is 01 April 2025 on a 12-month rolling contract. The set price the Council will pay for a completed Health Check will be £21.00. Providers using POCT will be required to meet consumable costs at their own expense. Providers using phlebotomy services will be required to meet these costs at their own expense. Providers will require the community tool kit at an approximate cost of £500 at their own expense. The service is available for AQP's who can deliver Health Checks face to face within Derbyshire to join at any time throughout the duration of the contract term.
