Description

The main purpose of the service is to deliver a service that fully complies with the statutory National Framework in relation to NHS Funded Care. This includes delivery of commissioned care, per individual patient, in line with service specifications that cover: Continuing healthcare Funded nursing care Complex care packages including palliative care Children's and young people's continuing care Specialist mental health care (Section 117) Specialist learning disabilities The service provides assessment, case management and review including: Allocation of care for patients based on clinical need Co-ordination of the continuing healthcare and funded nurse care panel Personal health budget planning Clinical review of continued eligibility for care Annual review of provider delivery The administration of referrals Continuing healthcare, mental health and learning disability and lead case management

Total Quantity or Scope

NHS Cheshire and Merseyside ICB has awarded a contract via Direct Award B to deliver a CHC service across Cheshire & Merseyside. The contract will start on the 01/04/2024 to 31/03/2025. The contract will be of nil value and will include individual place agreements within the contract that will include agreed costs. These are new providers delivering an existing CHC Service across Cheshire & Merseyside that were onboarded between 01/01/2024 to 05/02/2025.

** PREVIEW NOTICE, please check Find a Tender for full details. ** This is a Provider Selection Regime (PSR) confirmation of contract award notice. This contract has been awarded under the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. In accordance with the onboarding process the Cheshire & Merseyside ICB CHC Teams have identified a suitable provider to deliver CHC Services. The decisions to on board these providers were made by various Place CHC Teams from 01/01/2024 to 05/02/2025 to meet the immediate clinical needs of a patient. On the 12/03/2024 the Cheshire & Merseyside ICB Finance, Investment and Resource Committee approved the decision to award CHC contracts via Direct Award B from when PSRs were implemented. Two committee members have a standing declaration of interest. The decision to award was made in line with the conflict of interest policy.
