Description
The overall aim of the Virtual Ward and Clinical Outpatients Services Framework Agreement is to provide responsive and time limited support to individuals who are medically fit to receive treatment outside of an Acute hospital by providing defined clinical services and/or advice/information/signposting to inform a care pathway which enables patients to safely resettle at home and promote independent and choice. The service provider will facilitate appropriate interventions and provide pharmacy treatments at home or at outpatient clinics close to home. This will promote independence and reduce the likelihood of hospital readmission or long term care. The interventions will be prompt and provide access to services which enable timely discharge or admission avoidance.

Work closely with Participating Authorities to foster a positive working relationship with the goal being to maximise the effectiveness of appropriate evidence-based care. |

This procurement is at the pre-tender stage. The purpose of the PIN is to engage suppliers to understand market capability and capacity to deliver the requirements. It is anticipated that further meetings will then be arranged with suppliers to continue to develop market intelligence and refine the scope.
