Description
Surrey County Council are seeking tender submissions for Care Within the Home Services. These services will be procured through a Dynamic Purchasing System. The DPS will commence on 1st October 2021 and will be in place for two years with the option to extend up to a further four years (2+2).
Lot Division
1 | Home Based Care (HBC) Home Based Care includes Specialist Services for people with additional needs and future ambition for HBC Carers BreaksHome Based Care (HBC) covers the provision of personal care to individuals in their own home. The regulated activity ‘personal care’ involves supporting people in their homes (or where they are living at the time) with things like washing, bathing, or cleaning themselves, getting dressed or going to the toilet. |
2 | High Needs (Continuing Health Care; CHC) The CHC team commission High Needs packages for people who have complex health needs.The HBC complex care service will include the provision of trained care staff who have appropriate skills and competencies to meet an individual’s assessed health needs. The service may also include using specialist equipment and stock control of ancillary supplies, prescription management as required by the individual and set out in their care plan. Individuals may have a range of complex health needs and/or a significant disability and/or an additional mental health need that impacts upon their daily lives. The service may also be required to support families/carers. |
3 | Live-In Care Live-in care is a personal, full-time care service, usually delivered by a care worker who lives within the individual’s home. The nature of the care and support required will vary depending on individual circumstances. Some individuals may require a long-term service while others may only require live-in care as a short-term emergency intervention. |
4 | Services to People with Additional Needs (Social Care) Bidders will be asked to confirm if they would like to deliver home based care services to people with additional needs. |
5 | End of Life Care (EOLC) Placeholder Lot Please note, currently this is a placeholder lot and may become available as an opportunity over the life of the DPS.End of Life Care is also known as Palliative care).End of Life Care is defined as: ‘care that helps all those with advanced, progressive, incurable illness to live as well as possible until they die. It enables the supportive and palliative care needs of both patient and family to be identified and met throughout the last phase of life and into bereavement. It includes management of pain and other symptoms and provision of psychological, social, spiritual and practical support’ . Additional information: Please note this lot will not be available at the beginning of this DPS and is currently a placeholder for a future requirement which may become available under the DPS. The Council reserve the right to present this lot to bidders in the future, and qualification criteria and detailed specifications will be provided then. |
