Description

NHS NEL CCG (BHR Region) are looking for an organisation to work in partnership and collaboration with to develop training and digital systems to support the Domiciliary/Home Care workforce, to monitor the health of vulnerable older people in their own homes. The benefits of the service will be: • Reducing ED and Non Elective Admissions • Providing care closer to home • Supporting expert and enhanced home and domiciliary care approaches • Supporting primary care to monitor vulnerable patients in the community The successful bidder will work across BHR (potentially with 2 targeted boroughs) to develop: • A training programme and on-going support for domiciliary care staff • Provide digital apparatus to monitor patients • Have a research partner to evaluate the impact of the pilot • BHR are looking for a partner who could also match or identify other funding sources to support the overall delivery of the pilot The pilot will run for 12 months from the point of commencement, supporting approximately 350-400 patients. BHR are looking for the pilot to commence by 15st December 2021, and the estimated contract value per annum non recurrent will be £100,000 (including Project Management, research and care hours backfill).
