• to provide a home and community based short break daytime and evening service; • individual/small group support for activities outside the child/young person’s home; • a sitting service in the child’s home with the disabled child/young person; • short term practical support in the child/young person’s home at times of family pressure.

Total Quantity or Scope

Individual support for activities outside the child’s home which provides a short break for parent/s or carer and introduces the disabled child or young person to activities that they may not ordinarily have accessed. Where possible the child or young person shall be introduced to activities or groups which may be continued once the service has ended.

Renewal Options

Option to extend for 2 years which takes the contract to a total of 66 months.
