Description

The ASL shall comprise four Categories. The Categories under this ASL are: • Specialist home-based care and support for people with dementia (ages 18+); • End of Life (including Continuing Healthcare (CHC) Fasttrack) care and support (ages 18+); • Live-In Care and Support (ages 18+); • Nurse led home-based care and support (ages 18+).

Lot Division

1 Specialist home-based care and support for people with dementia (ages18+)
  • Value: £500K

The Authority is seeking to commission, across the Isle of Wight, care and support services which will improve the person’s quality of life and maximise their long-term independence when they are living at home with dementia. The delivery of this service will require close working with other health and care providers, including personal assistants and voluntary sector organisations. Every Supplier appointed on to the ASL under this Category shall provide an innovative, high quality service to support individuals, who wish to remain in their home but who require care and support to do so, supporting people with Dementia or other memory and cognition diagnosis of all ages. The Supplier will be responsible for: • The provision of timely, high quality care, including palliative and end of life care; • Meeting identified health needs for the individual; • Ensuring that people are treated as individuals and their preferences and choices are respected; • Implementing individual support plans in a way that promotes independence and confidence whilst ensuring that all individuals are treated with dignity and respect; • Use a strengths-based approach that embodies the principles of person centred and outcome focussed support; • Providing the people they support with a regular care worker (or small team of care workers) who understand and can meet their health and social care needs; • Contributing to the self-care and management of those people suffering with longer term conditions; • Responding flexibly to fluctuating health and social care needs, within the boundaries of this contract; • Work proactively with health and care partners to maximise the persons outcomes and reablement goals.

2 End of Life (including Continuing Healthcare (CHC) Fasttrack) care and support (ages 18+)

The Supplier shall provide an innovative, high quality service to support individuals, who wish to remain in their home but who require end of life care and support to do so. This service is a Domiciliary Care Service provided on an outreach or community basis and the service will operate and be available for referrals 24 hours a day, 7 days a week, every day of the year (including bank holidays). This service includes but is not limited to, the following activities: • Assistance with washing, bathing and showering; • Assistance with toileting; • Assistance with dressing and undressing, getting up and going to bed; • Care of skin, hair, teeth and nails, where appropriate; • Medication- checks and reminders; • Monitor the persons condition and risks and alert the GP/case Manager if concerned; • Food preparation at the person’s home; • Assistance with feeding; • On occasion additional and social supported living support may be required; and • Additional support, such as driving the individual to and from a medical appointment or accompanying them to a social event, as required.

3 Live-in Care and Support (ages 18+)
  • Value: £375K

The Authority is seeking to commission, across the Island, live-in care services which will the person’s quality of life and maximise their long-term independence. The delivery of this service will require close working with other health and care providers, including personal assistants and voluntary sector organisations. The Supplier shall provide an innovative, high quality service to support individuals, who wish to remain in their home but who require 24 hour a day care and support to do so, supporting people with the following care and support needs: • General care and support needs; • Physical disability; • a Learning Disability; • Dementia or other memory and cognition diagnosis; • In need of palliative and end of life care; • Specialist conditions such as brain injury, Motor Neurone Disease, etc.; • Mental Health

4 Nurse led home-based Care and Support (ages 18+)
  • Value: £2M

The Authority is seeking to commission, across the Island, nurse led care and support services which will improve the person’s quality of life and maximise their long-term independence. The delivery of this service will require close working with other health and care providers, including personal assistants and voluntary sector organisations. The Supplier shall provide an innovative, high quality service to support individuals, who wish to remain in their home but who require 24 hour a day care and support to do so, supporting people with the following care and support needs: • General care and support needs; • Physical disability; • a Learning Disability; • Dementia or other memory and cognition diagnosis; • In need of palliative and end of life care; • Specialist conditions such as brain injury, Motor Neurone Disease, etc.; • Mental Health

Renewal Options

The Term of the Approved Supplier List shall be 01 February 2023 (go live date) until 31 January 2029 with the option to extend for up to a further two years at the sole discretion of the Authority. The Authority intends to begin using the ASL for requirements as soon as the ASL goes live.

Other Information

In this procurement, the Lots shall be referred to as Categories. There will be no limit to the number of Suppliers admitted on to each Category. Any values listed in this Contract Notice are indicative annual values. All Supplier’s appointed on to the Authority’s ASL across all categories shall ensure that they are registered with the Care Quality Commission (CQC) at all times during the Term. In some cases, the Authority may need to procure a blended package of services covering one or more Categories. In this case, the Authority shall procure each package individually through the appropriate Lot and the successful suppliers will be expected to work together to ensure the successful delivery of the blended package. This is a non-exclusive ASL and the Authority reserves the right to use an alternative route to market during the life of the ASL if it is deemed to be appropriate. The ASL shall reopen for new Suppliers to join annually. A Contract Notice will be published annually on the anniversary of the Go Live date and will remain open for a minimum of 30 calendar days for Suppliers to submit their application to join the ASL. Suppliers will be assessed against the same criteria as is published in this document and the same pass marks shall apply.
